Remember that microbial theory of disease was just gaining traction at the time in the late 19th century. Pasteur, Lister, and others had started to make the connections but generally physicians held to the theories of Galen, who was, literally, as a Greek God to the profession. And don't forget that effective antibiotics wouldn't come into existence for another fifty years.
